Feedback, compliment or suggestion received.
Your compliment or suggestion will be sent to the Chief Executive Officer.
You will receive a response within one week or sooner if more urgent.
If you need help to make a complaint or your are not happy with the way we handled your complaint we can let you know some agencies that can assist you. Remember you have the right to have an independent advocate of your choice to support you when discussing services issues or when making a complaint.
Alternatively, you may wish to download the PDF feedback form. This form can then be posted to DDWA at 2 Delhi Street, West Perth WA 6005.
A complaint can be made to the NDIS Quality and Safeguards Commission, if Developmental Disability WA was unable to resolve your concern or complaint.
